The Enhanced Doctor
Chapter 907: Let Xu Yino Take the Lead Surgeon Role
(Thanks to firebat1980 for the encouragement with the monthly ticket)
"Teacher Liu, the heat stroke patient has stabilized quite a lot. However, Wu Zhen's post-surgery infection has worsened somewhat, and the indicators are not looking good."
Xu Yino reported upon her return.
Liu Banxia frowned. He initially planned to call Zhang Zhiyuan directly but decided to go over and check it out himself.
"I guessed you would come over; it's a normal infection. It's just that the surgery was quite major and still requires careful adjustment, but there is no threat to life," Zhang Zhiyuan replied.
"Sigh... as long as there are no other complications, I initially thought there might be liver failure," Liu Banxia said.
"The pathological results from the sampling point were quite good, but the current slow recovery will definitely affect the upcoming chemotherapy schedule, right?" π§πβ―β―π€β―π£πβ΄πππ.ππ°π
"Surely, otherwise the body wouldn't withstand it, and suitable medication combinations couldn't be provided," Zhang Zhiyuan said.
"His overall direction is still positive; yesterday's white blood cell count was a bit high, but after medication adjustments, it has come down today. Rest assured, it's still within controllable range."
"It's just uncertain whether he can withstand the upcoming chemotherapy, as everyone reacts differently to drugs. After all, he's a doctor himself, and he'll have his own decisions when the time comes."
Liu Banxia nodded. Although he is not an oncology doctor, he has always maintained a cautious attitude toward chemotherapy.
Mainly due to the failure of chemotherapy with Tiggie's owner.
There are many successful cases of chemotherapy, but also quite a few failures, which can ruin a family financially and make matters worse.
Coming out of the ICU, Liu Banxia's mood was still somewhat downcast. Even though Wu Zhen's prognosis is within normal standards, he hopes it could be better.
For Wu Zhen, his condition during surgery wasn't as severe as the instruments suggested, which was his fortune. But the post-surgery recovery process hits the edge of prognosis, proving a test for subsequent treatments.
Although his tumor hasn't spread to a large area, chemotherapy is still a necessary step for safety reasons.
If he can recover faster and become stronger, he can suffer less through the chemotherapy process with better outcomes.
Though chemotherapy medication has guidelines and there are no major deviations in drug combinations, there's still significant room for adjustment concerning dosages.
Chemotherapy to cure cancer has been on the forefront of medicine, and even with the most scientific and standard medication regimen, each individual patient may respond differently.
Xu Yino didn't dare say much more, as the postoperative prognosis for Liu Banxia's patients has usually been at middle to upper level. Many merely have some inflammation, but comparatively, this patient's recovery hasn't been ideal.
"Director Liu, shall we accept a patient? A knife wound emergency patient is being brought in shortly."
Just back in the hall, Qi Wentai greeted them.
"Alright, Xu Yino, get ready for admission," Liu Banxia said.
"Teacher Qi, what kind of wound is it?" Xu Yino asked.
"It's a stab wound to the abdomen, and the knife is still lodged there," Qi Wentai said.
Xu Yino nodded, preparing mentally.
After a while, the emergency vehicle arrived.
"Blood pressure 80/120, heart rate 110. Stab wound to the abdomen, no resident medical history. No history of hypertension, diabetes, or heart disease, consciousness is clear," said the paramedic.
"Rescue room one, have you already contacted the family?" Xu Yino asked.
"Contacted, and we have finished the call with my family," the patient replied.
"Don't worry too much; seeing your spirit so good, it likely hasn't hit any organs causing major bleeding. How did this happen?" Xu Yino asked as she pushed the patient.
"Just messing around with friends, and it accidentally stabbed when I fell," the patient said, his eyes somewhat evasive.
"Don't worry; we don't mind how it happened. It seems like a fruit knife and needs careful handling. Now, one, two, three, transfer to the bed, connect the ECG monitor, bedside X-ray," Xu Yino said as they reached rescue room one.
The interns who followed quickly began setting up.
"It doesn't look like anything major, but the knife's position is somewhat close to the stomach," Xu Yino said after the X-ray was taken.
"Blood routine, biochemistry, rapid blood typing, palpation reveals a soft abdomen, also perform an ultrasound, and alert the operating room for preparation."
After speaking, Xu Yino looked towards Liu Banxia.
Liu Banxia nodded, agreeing with her diagnosis and corresponding actions.
Even though no organs are affected, it's a fruit knife, and the blade might carry additional bacteria. For safety, it's better to remove the knife in the operating room and perform intra-abdominal irrigation and disinfection.
It's not like a movie; stabbing someone once, let alone ten or eight times, where the character can still run around freely without worrying about infection or complications.
"Teacher Liu, would you please take out the knife?" Xu Yino asked.
"You still need me here? Go sign the surgery consent form yourself." Liu Banxia said.
This is also a necessary procedure. Most emergency patients admitted are unconscious or have impaired awareness, making it impossible to communicate effectively. In such cases, they are directly sent to the operation room for rescue.
This patient is different; this patient is quite alert. Since the family members are not present, it's necessary to explain the surgical situation to them thoroughly.
Moreover, there are currently no symptoms of internal bleeding, so there is no need to rush them to the operation room.
"How about now, can you endure the pain?" Xu Yino asked the patient.
The patient nodded.
"Then bear with it for now. If we give you a painkiller, we'll have to adjust the anesthesia later. Anyway, you've gotten through the worst part, so don't worry too much." Xu Yino said.
Seeing that they were about to explain the surgery and extraction procedure, Liu Banxia didn't stay in the emergency room but instead walked outside.
As soon as he stepped out, Qi Wentai gave him a look, signaling him to pay attention to a hurriedly moving young woman in the waiting area.
"Just arrived?" Liu Banxia asked.
Qi Wentai nodded, "Is she related to the emergency patient who was just brought in? She's been looking this way but doesn't dare to come over."
"Let's pretend we didn't see her. Whatever the reason, the young guy hasn't mentioned anything." Liu Banxia replied.
"Teacher Liu, we need to start surgery immediately, the blood pressure has dropped. There's a small amount of fluid in the abdominal cavity, probably just started bleeding." Xu Yino shouted at this moment.
"Alright, let's head to the operating room." Liu Banxia responded, then looked toward Qi Wentai again.
"Keep an eye on it; he's contacted his family. The blade is close to the stomach; once the family arrives, make sure to explain."
Qi Wentai nodded.
When Xu Yino pushed the patient out, it was obvious that the young woman was watching closely but still didn't dare to approach. Upon seeing Liu Banxia's gaze, she quickly seated herself on a chair.
"Young people these days, who knows what they're thinking." Liu Banxia mumbled.
But there was no time to worry about the young woman; the surgery was the priority.
"Xu Yino as lead surgeon, Li Tianyang as first assistant." Liu Banxia said.
"What? Really going to let me lead the surgery?" Xu Yino was startled.
"You've practiced stitching for so long, now it's time to use it. What are you afraid of?" Liu Banxia glared at her.
Xu Yino didn't dare to speak up anymore, albeit feeling a bit nervous.
The stitching of the gastrointestinal tract requires a high level of skill, and having such a responsibility all at once felt heavy on Xu Yino's shoulders.
"What's wrong? Feeling nervous?" Liu Banxia asked as he saw Xu Yino with a tense face while scrubbing in.
"Not at all, nothing to be nervous about." Xu Yino replied boldly.
In truth, she was distracted, thinking about how exactly to proceed, so she replied habitually.
Liu Banxia found it amusing and didn't see Xu Yino's response as insubordination, rather finding it quite entertaining.
"You all pay attention; many surgeries have commonalities. Like this one today, don't just see it as trauma emergency care; it also shares principles with foreign body extraction and stomach perforation repair." Liu Banxia addressed the interns.
"Today, just focus on Xu Yino; it's a chance for her to shine. If she performs well, she'll finish the surgery smoothly. If not⦠well, midway we might have to swap out."
"And Li Tianyang, are you able to manage as the first assistant? This differs a bit from routine abdominal surgeries. If you're not confident, say so now."
"Teacher Liu, no problem, I've participated in similar surgeries before." Li Tianyang quickly responded.
Liu Banxia nodded in satisfaction, "I'll leave it to you then. Remember, my fundamental rule for interns or residents I guide is never to show off."
"There are many opportunities to practice surgery, but patients don't have as many chances. A reckless move on our part might cause minor suffering or, worse, cost a patient's life."
The interns and residents all nodded along while waiting to scrub in.
Liu Banxia's serious tone made it clear this was no joke. They shouldn't think that once they're inside, everything is fine; poor performance can still lead to being dismissed.
However, there was a bit of excitement amongst them. After all, this was a Level-2 surgery, and if the situation turned out to be severe, it could escalate to a Level-3.
And now that Liu Banxia dared to let Xu Yino lead and Li Tianyang assist, it certainly speaks volumes, doesn't it?
In the past, they've only heard about the numerous opportunities at the emergency center. But to not only have many opportunities but also be practically showered with them, it's exciting.
But they also knew, to gain more opportunities, their work must be top-notch. Xu Yino and her peers had already earned a notable reputation among last year's graduates.
