"Doctor, how is my wife?"

Having just come out of the emergency room, the patient's husband asked anxiously.

"We still need to wait for the blood test results. We suspect it's due to overexertion. Can you tell us about your wife's condition lately?" Xu Yino asked.

"She's been quite normal. She mentioned feeling a bit tired recently, maybe from taking care of the child. Our little one is quite a handful and runs off if you're not watching." The patient's husband replied.

"She took some medicine, and felt slightly better. But these past few days, it seems to have recurred. Recently, I've been cooking dinner after getting home. Just when we were about to eat, I heard a noise from the room, and she fell."

"Don't worry too much. Her vital signs are stable, and her heart and lung sounds are normal. She has a slight fever, and we'll confirm the medication after the blood test results come out." Xu Yino said.

"The head injury is nothing serious, just a scratch, no swelling. There's no need to worry too much. Do you have any other relatives here? You should send the child home; the hospital is too chaotic, not good for the baby."

"I've already contacted the family, they're on their way." The patient's husband said.

"Don't worry, stay calm. Once the results come out, we'll see if hospitalization is needed. We've notified the lab to expedite the processing, and the results will be out soon." Xu Yino reassured.

The patient's husband nodded quickly and approached the examination bed, holding his child.

"Teacher Liu, I'm quite puzzled about why the patient is sweating so much. Even with frailty, would it cause this much sweat?" Xu Yino asked.

"I've noticed that too, but there are many conditions that can cause sweating. The patient's breath sounds, heart sounds, sinus rhythm, and blood oxygen levels are all normal, so further examination isn't justified." Liu Banxia said.

Xu Yino nodded.

Having briefed Liu Banxia earlier, she knew that this patient didn't seem to just have a simple cold. Liu Banxia had likely noticed this as well, but due to unclear indicators, they still had to wait for the blood test results.

Because if it's just a cold, the patient's night sweats would be unexplained. This is unlike taking medicine and sweating it out to cure a cold. From home to the hospital, enough time had passed for the sweat to dry.

Moreover, due to the heavy sweating, she decided to administer fluid therapy.

A few minutes later, an elderly couple rushed in; they were the patient's family. The patient's husband explained the situation to them.

"Teacher Liu, can you make a guess?" Xu Yino asked again.

Liu Banxia looked at her helplessly, "How can I guess? Nothing is clear, impossible to guess."

"But you guessed that the baby might be in respiratory distress, you can guess this too." Xu Yino argued.

"The baby had some indicators. What indicators do we have now? The only things to consider are electrolyte imbalance and dehydration." Liu Banxia said.

"You've already administered fluids, but we still need to await the blood test results for electrolytes. Don't overthink it, once the results are out, everything will be clear."

Feeling a bit reluctant, Xu Yino couldn't say much. She had actually hoped Liu Banxia would have insights, but this time he wasn't quite helpful.

She pondered more, increasingly feeling the patient's sweat symptoms were unusual, but the stethoscope and ECG monitor showed real data.

Feeling somewhat unwilling, she intermittently glanced at Liu Banxia for reassurance.

"It's worrying. If it's not a cold, we might need to consider tumors." Liu Banxia said helplessly.

Xu Yino's gaze, like Tang Dou seeking treats, glanced at him occasionally, only for Tang Dou to look away when caught.

"Teacher Liu, really?" Xu Yino asked in surprise.

"It's one possible direction. Or it could be a cold causing frailty. But don't forget the patient's husband's words about recurrent cold symptoms." Liu Banxia said.

"Also, childcare is tiring, but the child seems to be over eighteen months old. Sleep should be normal, allowing the couple regular rest."

"Thus, my suspicion about tumor-related conditions affecting the immune system or autoimmune diseases seems reasonable."

"You could ask the patient's husband about recent pain symptoms, but I doubt he'd notice much."

Additionally, the patient's current symptoms resemble chronic illness, so discussion after blood results is appropriate. Quit looking at me, I've shared all my thoughts."

"Oh."

Feeling a bit wronged, Xu Yino responded before running to the laboratory, waiting for the results.

"Actually, after seeing many patients, you might end up like Xu Yino," continued Liu Banxia.

"Some call it a doctor's intuition. I'd describe it as reasonable doubt."

"But remember, while we may have reasonable suspicions in our hearts, we shouldn't share them with the patients or their families. Because without a confirmed diagnosis, we can only make judgments based on the current situation."

"Our suspicions, when heard by patients and their families, carry the same weight as a diagnosis. As doctors, we must be very careful when speaking with patients and their families."

"This is also a kind of protective shield for you, helping to avoid any unclear patient complaints. How did you both end up here? What have you brought?"

Liu Banxia directed these words at Qiu Mingyuan and Xiao Yue, who were walking leisurely towards him.

"How else? We received orders from Sister Qiao to deliver ribs and stir-fried peanuts to you, with a fish inside," Qiu Mingyuan said helplessly.

"That's not bad. Have you eaten yet? When you're done, join me for a bite in the cafeteria," Liu Banxia said with a smile.

"We couldn't eat at home, but I sneaked two pieces of ribs on the way here," Qiu Mingyuan said with a mischievous grin.

"Brother Liu, the parents of my classmate said they wanted to invite you for a meal whenever you're free," Xiao Yue said.

"A meal, huh? Better not. You know how busy I am now—just waiting to get off work and fly home," Liu Banxia replied.

"Haha, I know, but I still had to ask," Xiao Yue said with a smile.

"Alright, you two wait in the break room. Once we're done treating this patient, we'll go eat," Liu Banxia said.

"Give me a five-star praise," Qiu Mingyuan joked.

Liu Banxia didn't pay them any heed, feeling quite content inside. That's his wife, always thinking about him.

The other interns, however, were focused on pondering the words Liu Banxia had just said rather than paying attention to his interaction with Qiu Mingyuan.

The straightforward words were easy to understand, yet also signified certain risks in future consultations.

After a short while, Xu Yino hurried back and directly handed the examination report to Liu Banxia.

"Share it with them and give your assessment," Liu Banxia said after reading it carefully.

"The patient's blood test shows mild anemia, elevated white blood cells and platelet counts. We need to conduct further physical examinations, including manual diagnosis and abdominal ultrasound," Xu Yino quickly stated.

"Can any of you think of the reasons?" Liu Banxia asked the interns.

The interns suddenly became nervous, unsure of how to respond.

"In future, when I don't name anyone, it's an open discussion. Now, Xu Zhenkai, please answer," Liu Banxia instructed.

"There are many possible causes for anemia symptoms. The elevated white blood cells and platelets suggest there might be an infection focus in the patient's body," Xu Zhenkai replied.

"Does Liu Yiqing have anything to add?" Liu Banxia asked.

Liu Yiqing bit her lip and said, "Considering our earlier discussion and the patient's fatigue and sweating symptoms, plus the elevated platelet primary possibility, it might be related to a bone marrow disease."

Liu Yiqing's words startled the interns, which explained why both Xu Yino and Liu Banxia appeared concerned.

Bone marrow diseases aren't easily curable and are quite daunting. Although it's currently just a possibility, the potential consequences are dangerous.

"Actually, Xu Yino already gave a clear hint. She wants to do a manual diagnosis and abdominal ultrasound to check the patient's spleen," Liu Banxia continued.

"If there's spleen enlargement, that's a clear indication, making it easier for us to proceed with further checks. Xu Yino, take the female students for the examination."

Xu Yino nodded and led the group over for the examination.

This was also a lesson; it's not very convenient for male students to participate. Though the examination hadn't started yet, Liu Banxia had already roughly determined the condition. Fatigue and sweating, recurrent colds, and now the clear blood test results.

Though it hasn't reached the diagnostic gold standard, the tendency is strong.

In less than ten minutes, Xu Yino returned with the group, "The spleen is enlarged, surrounding lymph nodes are swollen, there's petechiae on the chest, and no abnormalities in the liver and other organs."

"You'll handle this case entirely. Including explaining it to the patient, and any subsequent examinations—I won't intervene," Liu Banxia said as he looked at her.

Xu Yino immediately felt a bit aggrieved, glanced at Liu Banxia, and walked away with her head down.

The patient's current condition is very much in line with primary myelofibrosis symptoms, which is a type of leukemia—next step is to do a bone marrow biopsy for confirmation.

Informing the patient's family about such a disease, even if it's just a suspicion, isn't easy.
